Recent literature has shown optimal residual meniscal width after saucerization and repair of discoid lateral meniscus. Using serial magnetic resonance imaging analysis, the authors defined the appropriate postoperative meniscal morphology. Notably, larger postoperative midbody widths (>13 mm) were associated with shrinkage and higher complication rates, challenging the idea that greater tissue preservation is beneficial. Instead, the study identifies a "functional target zone" of 9 to 13 mm, aligning closely with the native lateral meniscus. Despite convergence in meniscal width over time, increased complications in initially oversized resections suggest no long-term advantage to under-resection. The findings also highlight the role of postoperative remodeling, meniscal extrusion, and patient activity levels in influencing outcomes. Overall, the focus has shifted from under-resection to optimal resection, providing clinically relevant guidance for improving outcomes in discoid lateral meniscus treatment.
