Anterior cruciate ligament repair has re-emerged as a potential treatment option in carefully selected patients; however, its role in contemporary practice remains uncertain. Although recent evidence suggests that surgical timing may not significantly influence short-term outcomes, this perspective should be interpreted with caution. The success of anterior cruciate ligament repair appears to be driven primarily by biological factors, including tear pattern and tissue quality, rather than timing alone. Importantly, outcomes in the literature are derived from highly selected patient populations, limiting generalizability. Concerns regarding durability, particularly in younger and higher-demand individuals, persist, and long-term data remain limited. The absence of comparative studies further restricts definitive conclusions regarding its equivalence to established treatment strategies. Current evidence may support the feasibility of anterior cruciate ligament repair under favorable conditions, but does not justify expansion of its indications. Until consistent long-term outcomes are shown across broader patient populations, established surgical approaches should remain the standard of care.
